State Certification Information
Certifying Body
In Michigan, the Michigan Department of Health and Human Services is responsible for the certification of CHWs in the state. This certification process is designed to standardize the qualifications of CHWs to ensure that they are adequately prepared to meet the community's health needs.
To qualify for delivering Medicaid-reimbursable CHW services, an individual must meet the following criteria:
Age Requirement
Be at least 18 Years of Age
Educational Requirement
Must have a high school diploma or equivalent certification.
Training Criteria
Must fulfill the CHW Training Program Pathway or Work Experience Pathway requirements.
Registry Application
Must successfully apply to the MI Medicaid CHW Registry and maintain good standing.
Medicaid Enrollment
Must complete the Medicaid provider enrollment through the Community Health Automated Processing System (CHAMPS).

You must obtain a Type 1 National Provider Identifier (NPI) as a Rendering/Servicing-Only Provider. Instructions can be found on the National Plan & Provider Enumeration System webpage.
